Application Instructions

DEGREE PROGRAM APPLICATION PROCEDURE

Department application deadline for Programs beginning Fall 2013:

Domestic:   December 31, 2012

International: December 1, 2012

We recommend submitting your application (in full) as early as possible. We begin accepting application forms September 1 and all application materials are due by the above deadlines for admission beginning the following Fall. 

Application instructions:

  1. Review Program Requirements:   Graduate Program
  2. Submission of the online Admission form. CLICK THIS LINK TO THE ONLINE APPLICATION FORM and payment of application fee. 
  3. Three References: the online application form will have reference forms that your referees can upload electronically.
  4. NOTE: you must upload both your CV and your personal statement in the online application form.
  5. Official GRE, GMAT or MCAT scores. (Scores are to be sent to the University test center University code 4853).
  6. Department Supplementary Application form.  The completed form can be submitted as a pdf attachment emailed to bmi-admissions@lists.utah.edu 
  7. Information for International Applicants.

CERTIFICATE PROGRAM APPLICATION PROCEDURE

We recommend submitting your application (in full) as early as possible. All application materials are due by:

  • July 1st for admission beginning in the Fall 2012 (although applications for Admissions received after April 1 will incur a $30 late fee)
  • November 1st for admission beginning in Spring 2013

Application instructions:

  1. Review Program Requirements:   Certificate Program
  2. Application: Submission of the Department Application Form 
  3. Transcripts: Submit scanned copies of transcripts by email along with your application form in item #2.
  4. References: After completing the top section and signing, send this reference form to two academic references to complete. The completed form can be submitted as a pdf attachment emailed to kate.handziuk@utah.edu. 
  5. Department Supplementary Application form.  The completed form can be submitted as a pdf attachment emailed to bmi-admissions@lists.utah.edu 
  6. Once you are advised by the Department that you have been accepted to the Graduate Certificate Program, you will then submit an application for Admission to the University and pay the application fee.  Do not apply for admission prior to being advised of acceptance to the Certificate program.
    1. On the University application for undergraduate admission, choose Second Bachelor's as your program of study.  Link here to application instructions and form
    2. List your chosen major as Undeclared.
