Can you bill the patient's insurance for labs?

We are sorry, the Branch Lab cannot bill the patient's insurance for lab work. You have the options of having the referring physician's office or laboratory billed or including a check or credit card number from the patient with the blood sample.



 Complete the HCFA form enclosed in your bill from the Branch Lab with the following information:

  1. In box 1a type or write in your insurance ID number
  2. In box 3, type or write in your date of birth
  3. In box 4, type or write in the policy holder's name
  4. In box 2, type or write in your name
  5. In box 5, type or write in your address
  6. In box 13, type or write in "Pay to Patient"
  7. In box 17, please indicate the name of your physician who referred you to the Branch Lab
  8. In box 21, please list your diagnosis code. You will have to obtain these from your physician's office
  9. In box 24, type or write in "90" on line 1 and line 2


Can a sample be sent at room temperature?

Yes. Just make certain that the sample is received by the Branch Lab within 24 hours of the draw time. If it cannot be shipped within that time frame, the sample should be frozen, then shipped on dry ice to preserve the integrity of the sample.
