| Crystal has been with the Rocky Mountain Center for Occupational and Environmental Health in the Continuing Education Department since 2010. She works with occupational safety and environmental health professionals coordinate registrations and tuition for various continuing education courses. Crystal is involved with monitoring OSHA-Approved Outreach Trainers in their efforts to provide a safe work environment for workers through necessary OSHA training and education. She grew up in Seattle, WA and moved to Salt Lake City as a teenager but quickly welcomed the move once she was able to experience the four seasons after being used to nothing but rain. Crystal enjoys the outdoors and staying very active especially during the warmer months when she can take advantage of fishing trips with her father. She loves spending time with her family and her adopted dog Bella, enjoys physical fitness, film and the arts. | |||

| Melody grew up in upstate New York but has called the west coast home for the past 13 years. Melody started working at UPAP in August 2006 as the clinical academic coordinator. She works with Nadia and Olivia assisting with both the 1st and 2nd year clinical rotations. This includes arranging student privileges so they can go with their preceptors to area hospitals, setting up rotation schedules with coordinators at clinical sites, arranging affiliation agreements with new clinical sites. In addition Melody is responsible for processing student and preceptor evaluation and serves as the go to for students out at clinical rotations.
Melody also processes the faculty and adjunct faculty appointments, and coordinates all of the clinical year celebrations such as return visits, white coat ceremony and graduation. While not working, Melody enjoys spending time with her husband, Cale and their recently adopted puppy, Sasha. She loves to cook, garden, and spend time in the beautiful Utah backcountry. |

| Mr. Bonnette is the Department "IT guy". Hailing from the Buckeye State, Larry graduated from the University of Utah with a degree in Mathematics and a minor in Physics. His varied background with the University includes supervising intramural sports, instructing classes in physics and basic computer skills, managing the campus movie theatre and coaching the University bowling team. Larry has been with the Department of Family and Preventive Medicine since 1995. | |||

| Karen is the Program Coordinator for the Occupational Medicine Residency Program at the Rocky Mountain Center for Occupational and Environmental Health. She enjoys getting to know the residents during their two year residency program. Karen grew up in Phoenix, Arizona. She likes to walk in Research Park and is happy to be living in Salt Lake City with its tall trees and four seasons. Besides walking, her hobbies include folkdancing and yoga. Karen began with the Rocky Mountain Center in 1995. | |||

| Heather has been with University of Utah Physician Assistant Program since June of 2005, first as Executive Secretary, and taking on Academic Coordinator duties shortly thereafter. Her primary responsibilities at UPAP include managing graduate paperwork, assisting with weekly student tutorial groups, administering annual alumni surveys and overseeing the alumni database, coordinating annual Master’s project presentations, and organizing the summer Clinical Practicum. She also handles logistics of the Post-Graduate Program and Evidence-Based Medicine Seminars, working closely with our Director and Associate Director of Graduate Studies. In addition, Heather represented the program as a member of the Department of Family Preventive Medicine staff development committee from 2006-2010.
Heather received her Bachelor of Science Degree from the University of Utah in 2010. While not working, Heather enjoys traveling, boating, hiking and spending time with her family and friends. |

She currently works with Dr. Stanford in the Office of Cooperative Reproductive Health as the Participant Coordinator of the CEIBA Study. Her interests include health policy, health information technology, and women's health. She enjoys reading, swimming, rock climbing, and rowing. | |||

Toni was born in Murray, Utah but was raised in Boise, Idaho, the second child of six. After graduating from high school, she went on to Cosmetology, graduating in 1978. She worked as a Cosmetologist until she married and moved to Southern California so her husband could attend Mortuary College. Her family moved to Utah in 1995. Toni left the comforts of a very quiet job that she has done off and on in the funeral industry for 17 years, to expand her horizons and knowledge. She joined RMCOEH in May 2006, where she started as the executive secretary for Drs. Kurt Hegmann and Moser, and is now pretty much the secretary for the Rocky Mountain Center. She has a family of one husband, four children, two dogs and a cat. Her family is her pride and joy, just ask her! Her hobbies are: gardening, hiking, camping, traveling and scrapbooking all those activities. |

| I was born into a large family that provided lots of fun and great childhood memories. I have a twin brother and four sisters. I grew up in Salt Lake City and graduated from Olympus High School. I attended the University of Utah for two years and University of Leeds in England for one year studying sociology and economics. I took the summer after my studies in England to travel around Europe, Morocco, and East Germany. My first job was with the Newspaper Agency during which time I married my high school sweetheart, Steve. I worked to help support us until he graduated from the University of Utah and took a sales job in Ogden and then in Oklahoma City. We eventually moved back to Utah and built a home in West Jordan. The next several years I enjoyed being a busy Mom to our six children. In 1991, I went back to work full-time in secretarial, bookkeeping and accounting positions. In October 1996 I came to the University of Utah as secretary to Dr. Royce Moser for seven years. In March 2003, I took the position of managing the finances for The Rocky Mountain Center. Today, I still enjoy my work and my associations with the people at RMCOEH and DFPM. I mostly enjoy spending time with my husband and family -- especially my nine adorable grandchildren. I also enjoy walking, reading, listening to music, traveling, doing activities with special needs Young Women and visiting with friends. |

| Connie Crandall is the Director of the Continuing Education Program at the Rocky Mountain Center for Occupational and Environmental Health, a position she has held since 1985. She also oversees the Hazardous Substances Training Program as well as the activities of the Mountain West OSHA Education Center. She works closely with both Center and adjunct faculty to provide over 75 programs annually consisting of both short courses and conferences. She received her MA degree from the University of Utah in 1976 and her MBA from the University of Phoenix in 1991. | |||

| Doris is the Director of Admissions and has been with the PA program since 2005. She is a graduate of the University of Utah with a BA in Political Science and a Master of Public Administration.
As chair of the faculty admissions committee, she is responsible for applicant advising and oversight the student selection process, from application evaluation though interview. She has served as a member of the CASPA (Central Application Service for Physician Assistants) Advisory Committee, a standing committee of the national Physician Assistant Education Association. In her spare time, she loves to cook, travel, garden, and play outside. |

| Julie was born in Reading, England, and when her parents migrated to Utah, 2 year old Julie had to come along as well. She has been with the Family and Preventive Medicine Department since January 1991. Since with the department, she has worked on such research projects as the Lung Cancer/Radon Study, Natural Family Planning (NFP) Study, and the Thyroid Cohort Study. Presently Julie assists the IT computer professional for the department, as well as developing and maintaining all department webpages.
Julie enjoys spending time with her family and playing with her two adorable Bichons, Allie and Lea. She loves to travel and see the beauties of nature, but only second to her love of football, particularly the Tennessee Titans. |

| Born and raised in western Canada, Joy has lived in Utah for twenty-five years. Her professional background includes experience in business, government, and education. While in Canada, she owned and managed a charter bus company and travel agency arranging tours throughout North America. After moving to Utah, she worked in the Office of Legislative Research and General Counsel for eight years and then moved on to the Utah Retirement Systems as administrative assistant to the director. After graduating from the University of Utah in Elementary Education, she taught in the Jordan School District for several years.
Joy joined UPAP in March 2009 and assists both the Utah Academy of Physician Assistants and the Utah Physician Assistants Program. Her UAPA responsibilities include working with the Executive Director on membership management and development, legislation, board meetings, conferences, AAPA constituent affairs, and other activities associated with managing the Academy. She also provides assistance to the UPAP in admissions and student related matters. Two sons and four grandchildren give her opportunity to enjoy vacations in California and Europe. Active in church and humanitarian efforts, she also enjoys reading a good book, hiking a scenic trail, planting a garden, and the company of family, friends, and pets. |

| Ulrike Ott was born in northern Germany and moved to Salt Lake City in 2004 in order to complete the Department of Family and Preventive Medicine's Public Health Program at the University of Utah. She earned her Bachelor's degree in Nutrition Science from the Christian Albrechts University in Kiel/ Germany. Ulrike is interested in international Public Health and has completed an intensive four week internship in Thailand which broadened her spectrum of international Public Health issues. Moreover, she currently volunteers for the International Rescue Committee (IRC) mentoring refugee families in their daily activities and needs.
She works as a Study Coordinator for the Rocky Mountain Center of Occupational and Environmental Health (RMCOEH) and is involved in a variety of projects such as coordinating literature reviews for the American College of Occupational and Environmental Medicine. Furthermore, she is training and supervising six research assistants/ work study students; gathers, organizes and analyses data and tries to cope with the minimal space availability at the RMCOEH. Most of all Ulrike enjoys traveling, hiking, whitewater rafting, racquetball and jogging. |

| Bob is the staff advisor to the Utah Rural Outreach Program. This outreach program sends medical students to rural communities to introduce health care professions to thousands of rural students each year.
Bob has a B.S. in Behavioral Science and Health and a Masters of Public Administration. Bob has interest in the development of educational Web based programs using the Internet. He has coordinated a statewide health professions recruitment and retention program, managed a private health care clinic, developed and managed youth programs as well as worked as a respiratory therapist at two major hospitals. |

| Leslie has been with the Department of Family & Preventive Medicine since 1988. The first eight years were spent as a staff member in the Family Practice Division and Family Practice Residency program. For six years she was the Clinical Coordinator for the Utah Area Health Education Centers (AHEC) in DFPM. She is involved in the planning and implementation of MDID 7030 and MDID 7060 Social Medicine (a course for first-year medical students). This includes the “service-learning” community practicum. She coordinates the rural and underserved student experience placements for nurse practitioners, physician assistants, pharmacy, social work, occupational therapy, physical therapy, dental, third- year family practice clerkship medical students and primary care residents with Utah AHEC and the SEARCH program. Leslie is also a member of the Staff Development Committee. | |||

| Riann has worked for the Rocky Mountain Center for Occupational and Environmental Health since 2007. Riann has been working as a research coordinator on the University of Utah’s truck driver study among other occupational related epidemiology research. In addition, Riann has worked extensively on the centers American College of Occupational and Environmental Health practice guidelines. She is a recent graduate of the University of Utah with a bachelor’s degree in exercise and sports science and has plans of pursuing further education in medicine and research. She grew up in the Salt Lake Valley and has been an avid skier since the age of 3. Her hobbies include yoga, running, pastel painting, and hiking. | |||

| Jessica Sanders currently works as a research assistant in the Office of Cooperative Reproductive Health (ORCH). Jessica has worked with Dr. Stanford since 2009. Among other ORCH responsibilities, she currently serves as the Participant Coordinator for the ongoing NIH funded Fertility Experiences Study (FES). Jessica graduated from Colorado College with a Bachelor's degree in Neuroscience. After working as a community health educator in Austin, Texas, she decided to move with her husband to his hometown of Salt Lake City, UT and pursue a degree in public health. Jessica will receive her Master’s in Science in Public Health from the University of Utah in 2011(pending defense August 2nd). Her graduate research is focusing on the analysis of the Pregnancy Risk Assessment Survey (PRAMS) data and the use of assisted reproductive technology (ART) in Utah.
Jessica will be starting the Doctoral program in Public Health at the University of Utah during the Fall 2011 and plans to focus her research on Maternal and Childhood Health with an emphasis in population studies and Reproductive and Perinatal Epidemiology. When not at the DFPM, Jessica enjoys camping and road biking during the warmer months and skiing (touring, downhill, and cross-country) when there's snow. |

| Originally from Minnesota, Karen has lived in Utah for the past fifteen years. Her educational background includes undergraduate degrees in English (Carleton College, MN) and biology (University of Utah) and a Master of Science degree in public health (University of Utah). She began studying for the doctorate degree in public health in the fall of 2007, with hopes to graduate in the spring of 2011. Broadly interested in the epidemiology of pregnancy, infancy and childhood, Karen’s specific research background includes evaluating energy balance among postpartum adolescents, examining the competitive food environment of Utah’s largest school district, and assessing the development and implementation of school wellness policies within Utah schools. She currently works as a research assistant for Dr Joseph Stanford, helping him with various aspects of his research including data analysis and the preparation of scientific papers for journal submission. Karen enjoys spending her leisure time cross-country skiing and backpacking with her ten-year-old daughter and husband. | |||

She provides administrative support to the program, including coordinating WATS and Survival Skills, tracking residents' and fellows' duty hours and faculty time away. Amy earned a BA in Wilderness and Leadership in 2004 and also completed coursework in Ornamental Horticulture before moving to Salt Lake City from Grand Rapids, Michigan in August of 2008. She joined the Department of Family and Preventive Medicine in 1987 initially in Medical Student Education and in 1991 with the Family Medicine Residency Program where her primary responsibility is recruitment. | Nirupma Singh is a graduate in Dental Surgery. She is currently pursuing a Masters program in Public Health at the University of Utah. She is working as a Graduate Research Assistant for Dr.Joseph.B. Stanford. She supports the ideology that reforms in the health care sector particularly the reproductive and family planning issues can be achieved largely by empowering women. Given an opportunity, she would like to work as a champion for the women’s rights in Southeast Asia and Sub Saharan Africa. | |||

She worked at the University of Utah in the School of Business for five years, and since 1999 in the Department of Family and Preventive Medicine as an Administrative Assistant. She currently works in the Chair’s office and with the Health Studies Fund. Julia is a wife, mother of four sons, and grandmother of one grandson and one granddaughter. She is a quilt maker and enjoys reading and road trips with her husband. | |||

| Margie received her Bachelor of Arts degree from the University of Utah in Speech Pathology. After working several years in the field of ophthalmology, including the National Collaborative Diabetic Retinopathy Study at the University of Utah, she worked nine years at Holy Cross Hospital as a health services coordinator, house staff coordinator and medical staff liaison. She began working in the Department of Family and Preventive Medicine in 1992, with the implementation of the required third-year Family Medicine Clerkship into the medical school's curriculum. Currently she is the Manager of Medical Student Education in Family Medicine. | |||

| Sharon Talboys, MPH is the Associate Director of the Office of Public Health Workforce Excellence, which aims to bridge the educational gap between academics and practice. She also works on global health projects and coordinates study abroad activities in India. She earned a Masters of Public Health degree in 1998 from Emory University’s Rollins School of Public Health with a focus on international health and behavioral science. In 1995, she received a Bachelor of Arts degree in Anthropology from the State University of New York at Geneseo. Ms. Talboys came to the University from the Utah Department of Health where she worked for over 10 years on issues ranging from maternal and child health, health information systems, workforce development, and preparedness. For most of her time at UDOH she held management positions including Training and Exercise Project Director and Deputy Director for Emergency Medical Services and Preparedness. | |||

| Janet has worked for the Department of Family and Preventive Medicine with the Rocky Mountain Center for Occupational and Environmental Medicine since October 2000. She has worked in the health care field since high school graduation and received her education in Southern California. She functions as assistant to the faculty physicians by performing and overseeing both office responsibilities and clinic related duties. She is responsible to oversee most clinic support activities, and also completes initial patient billing process such as preparing charge sheets with procedure and diagnosis codes and entering charges into the computer data base. She makes recommendations regarding exam pricing and total care pricing and develops client’s contracts. Hobbies include: Hiking, motorcycle riding, babysitting my grandchildren, and playing Mrs. Santa Claus. | |||

| Tina Elton-Reese Waters has worked for the University of Utah School of Medicine, Department of Family and Preventive Medicine administration since 2002, as the director of development and communication and also director of the office of the chairman. She previously worked in the Dean's Office in the University of Utah School of Medicine as a program coordinator within the office of student affairs. Her love of education currently includes a Masters Degree in Public Administration from the University of Utah, Bachelors of Science in Mass Communication with an emphasis in Public Relations from the University of Utah, and an Associate Degree in Commercial Art. She has served on numerous University committees, having served as president-elect, then president of the University's Staff Council. She is currently serving on the executive committee for the University's Presidential Commission for the Status of Women. Tina's challenges each year include balancing time among a large multi-discipline University department with her family and many hobbies and interests. A boating and equestrian enthusiast, she also finds relaxation in caring for her horses, pampering her garden flowers, water coloring, photography, hiking, and reading (particularly history). She and her husband Blake are the proud parents of four sons and two daughter-in-laws who continue to fill the chapters of their lives with great adventures, continuous laughter, and best of all, grandchildren. | |||

| Hina joined the Department of Family and Preventive Medicine in 2009 as an Office Assistant while also attending school at the University of Utah. She provides secretarial and administrative support for the department and is the point of contact for greeting and assisting students and visitors. She recently graduated with her master’s degrees in Public Health and Healthcare Administration. While enjoying her new free time, she is currently working to finish a research project with the Division of Infectious Diseases as well as look for more permanent work in her field of study. Hina loves to travel, cook, and spend time with friends and family. | |||
