Research Application
Thank you for your interest in conducting research through the Utah Health Research Network (UHRN). The purpose of the UHRN is to foster research in the network by facilitating researchers’ access to patients and data, while also developing and supporting network-driven research projects that will benefit clinical practice and advance knowledge in primary care and/or public health.
Currently the UHRN includes 9 clinics, 127 clinicians, providing healthcare services for more than 300,000 patient visits per year. The Community Clinics (CC) offers a wide rage of specialties of care along with the foundation of family medicine, internal medicine, pediatrics, and OB/GYN services. During fiscal year 2005- 2006 the CCs saw 313,341 visits of which 61.5% were by female patients and nearly a third (29.9%) was patients between the ages of 0 and 19 years. Since this data was reported two new clinics have opened, one in Centerville and a “quick” clinic in the Dan’s Grocery Store in Olympus Cove. One additional clinic is due to open in Daybreak during 2007.
The UHRN offers services that we can provide to researchers from a dedicated Community Practice Enhancement and Research Coordinator (PERC), IRB application and submission assistance, grants administration and review, and manuscript editing. Each service is billed at $50.00/hour and we will negotiate hours per week needed for each service prior to work starting.

- Preliminary Project Approval Form (PPAF) –must be approved by the Board before your project begins
- Fee for Service description and schedule
- Community Clinic Map
- UHRN Application Approval Flow Sheet
Please fill out your PPAF and return to susanne.cusick@utah.edu. You will then be assigned a Participating Research Committee member who will work with you to get your project through the approval process.
