Graduation and Forms

All graduate candidates for graduation must submit a graduation application to the Registrar’s Office, Graduation Division and a Candidacy form to their academic advisor. Deadlines for submission are listed below. Applications will not be accepted more than one year in advance.

Required forms for graduation:

Graduation Application – The graduation application must be submitted to the Registrar’s Office, Graduation Division before the indicated deadlines above. A copy of this form must also be turned into the academic advisor for the students’ academic file. Please note, graduation applications will only be accepted by the Registrar’s Office if submitted using the students U-mail or by bringing it to Student Services Bldg., Window 15.

Master Candidacy Form – The Master candidacy form must be signed by the student’s chair and committee members before being submitted to the students’ academic advisor. This form requires that all courses counting towards the desired degree be listed on the form.

PhD Candidacy Form - The PhD candidacy form must be signed by the student’s chair and committee members before being submitted to the students’ academic advisor. This form requires that all courses counting towards the desired degree be listed on the form. 

Additional Forms:

Independent Study – The independent study contract must be completed and returned to the academic advisor before a permission code will be distributed for registration. The contract requires an outline of the objectives of the study, a summary of the topic, papers/projects planned to be completed, reading lists, and designated meeting times with the faculty mentor.

Core Course Waiver/Substitution – The core course waiver or substitution is used to facilitate a potential course waiver or request for core substitution. Please complete the form in its entirety with the appropriate supplemental documents and signatures.
