Criminal Background Check
The School of Medicine requires successful completion of a criminal background check for all new School of Medicine faculty appointments. The University of Utah uses a Consumer Reporting Agency to perform a Criminal History Background Check as part of the employment process. The Consumer Reporting Agency conducts an investigation that verifies your social security number, obtains information regarding past employment, and performs a criminal background check.
The Consumer Reporting Agency will track your counties of residence to search for criminal records. Before an applicant is ever denied employment based on information obtained in the report she or he receives a copy of the report and a written description of his or her rights under the Fair Credit Reporting Act.
Applicants must complete a criminal background authorization form along with offer acceptance.
The University of Utah has changed vendors for background checks and is now using Accurate Backgrounds. Effectively immediately, you will no longer need to fill out the criminal background check form that we have used in the past.
Please download the letter of instruction to be sent to candidates along with their letter of offer. Once a candidate has accepted an offer, the department administrative assistant/hiring manager will need to click on this link : http://www.hr.utah.edu/openposition, enter candidate information in the online form. HR will initiate the check and department will be notified when clear to hire. Once you have initiated the request through the HR link, candidates will be sent an email asking them to go online and complete the background authorization form electronically. It is critical that you let the candidate know that the hiring process will not go forward until they have completed this online information to initiate the background check.
Once the candidate has entered their information, the background will be processed. You will be notified by HR as soon as the background is completed. Once you receive this notification, you will need to save as a PDF file to be uploaded to the appointment EPAF.
We have outlined the new process below:
* Hiring departments will no longer print out the University of Utah Consent Form (Background Release).
* You will now provide the new hire candidate with the Information Letter, which is now available on our website http://www.hr.utah.edu/openposition and titled Criminal Background Check ONLINE Process. It is critical that you let the candidate know that the hiring process will not go forward until they have completed this online information to initiate the background check.
* Departments will then provide your HR Representative with the following information:

* The background check will be initiated by your HR Representative. The candidate will be sent an e-mail with instructions to log in and complete the background authorization form online within a specific period of time.
* You will be notified by your HR Representative when the candidate has been cleared for hire.
