Application Process
Financial aid applications (including federal and University applications described below) are available online in January of each year and are due before April 1. Applicants may apply prior to admission and are encouraged to do so (see filing deadlines below).
Applications
Federal Application (FAFSA or Renewal FAFSA)
You may apply by using the Internet at www.fafsa.ed.gov
Please complete ALL information on the FAFSA, including parent's information.
When answering the questions in Section 8 of the FAFSA or the Renewal FAFSA, pay close attention to the following:
When you list the universities you plan to attend, you must indicate where you will be living while attending that university. For example, if you list the University of Utah, indicate where you will live, using the Housing Codes listed on that section of the application.
Please Note:
- The University of Utah and its Title IV Code, which is 003675, must be listed.
- The University of Utah address is:

Salt Lake City, UT 84112-9055. - If the University of Utah is preprinted on the Renewal FAFSA, be sure to check the "Yes" box to release information to the same colleges you listed in 2006-2007. If it is NOT preprinted add it under new colleges for 2007-2008.
UFORM
Complete the online Institutional Financial Aid Form.
School of Medicine Supplemental Application for Scholarships and Loans
In order to be considered for the private loans and scholarships available through the School of Medicine, students must complete the Supplemental Application by April 1.
Filing Deadlines
The ideal file completion date for need-based financial aid is April 1 (formerly known as Priority filing) of the calendar year prior to the academic year for which you want aid. (Example: For aid during the 2009-2010 academic year, one must have a complete file by April 1, 2009.) A complete file has no incomplete checklist items, (no missing information) as determined by the Office of Financial Aid and Scholarships. Please allow for an average processing time of 2-3 weeks. Information submitted March 31st would likely not be considered complete.
Late FAFSA and UFORM applications are accepted and considered, depending on adequate funding and processing time. Most aid programs have limited funding; applicants who have a complete file by the ideal file completion date are considered first for such aid.
Some forms of financial aid are available year-round. If you do not apply for need-based aid before the ideal file completion date, you may still do so at any time during the academic year, as long as the information from the federal processor reaches the University of Utah before the last day of class for the semester that you are requesting aid. Applications can take time to process and must be sent to the federal processor at least 120 days before the beginning of the semester if financial aid is to be available for tuition deadlines.

Verification of Information
You may be required to submit a signed and complete copy of your own and/or your spouse's 2008 Tax Form 1040, 1040A, or 1040EZ, including W-2's with all schedules. Only students who are requested by the University of Utah Financial Aid Office to provide additional information need to do so. To avoid delays in processing their application, such students should respond immediately when asked to submit these documents. Federal funds cannot be disbursed until required verification is completed. Keep copies of your tax forms, W-2's and all schedules!
Notification/Acceptance Letter
The University will notify you via e-mail or a postcard when you have been awarded Federal Aid. This notice will give you directions to go to your Campus Information System website to review your Financial Aid Award Letter. If you will be accepting a Federal loan, you will need to accept the award via the website.
Loan Orientation
A loan orientation must be completed each year you receive a loan. Completion of this computer-tutor program is required prior to receiving Stafford, Perkins and Institutional loan funds. If you have been awarded an Institutional Loan through the School of Medicine, you must complete the Perkins Orientation.
