Financial Aid Checklist
- Complete student, and/or spouse U.S. Income Tax Form 1040, 1040A, or 1040EZ as soon after January 1, as possible. Most information on the FAFSA or Renewal FAFSA is based upon tax data.
- Complete Free Application for Federal Student Aid (FAFSA).
- Review Student Aid Report from the Federal Processor and submit any changes/corrections as soon as possible.
- Complete UFORM on-line Institutional Financial Aid Form by March 15.
- Complete the Supplemental Application for Institutional Loan and Scholarships before April 15.
- Check to see if your file has been selected for Verification (this will usually take place around the end of April or beginning of May). If your file is selected you will be required to submit a signed and complete copy of your own and/or your spouse's 2006 taxes and W-2's. Your financial aid file will not be packages with awards until you have completed the verification process.
- Once your financial aid status is complete, view and complete your Financial Aid Award Letter.
- Complete a Dependent Care Form if you want to increase your cost of attendance to cover any additional child care expenses. Forms will be available in the Medical Education Office (Room 1C102) or in the Financial Aid Office (105 Student Services Blds.)
- Complete an on-line Student Loan Orientation (one for the Federal Stafford Loan and one for the Federal Perkins Loan).
- Complete a Master Promissory Note for, subsidized and/or unsubsidized Stafford loans, that will be mailed to you or made available online around the beginning of August. Mail this promissory note back to the guarantee agency (i.e. UHEAA, Medloans, etc.)
- Complete a Perkins Loan promissory note and return it to the University of Utah Financial Aid Office (105 Student Services Bldg.)
- Complete a Parental Data Collection Form and turn in copies of your parents' tax returns (only required for consideration of Primary Care Loan) before May 15.
- Maintain a copy of all documents for your personal file.
