Financial Aid Checklist
- Complete student, and/or spouse U.S. Income Tax Form 1040, 1040A, or 1040EZ as soon after January 1, as possible. Most information on the FAFSA or Renewal FAFSA is based upon tax data.
- Complete Free Application for Federal Student Aid (FAFSA).
- Review Student Aid Report from the Federal Processor and submit any changes/corrections as soon as possible.
- Complete Financial Aid Institutional From (UFORM) by March 1.
- Complete the Supplemental Application for Institutional Loans and Scholarships by April 1.
- Check to see if your file has been selected for Verification by accessing the Campus Information System (this will usually take place around the first week of March). If your file is selected you will be required to submit a signed and complete copy of your own and/or your spouse's 2010 taxes and W-2's and a 2011-12 Independent Verification form. Your financial aid file will not be packaged with awards until you have completed the verification process.
- Once your financial aid status is complete, view and complete your Financial Aid Notification (Award Letter).
- Complete a Dependent Care Form if you want to increase your cost of attendance to cover any additional child care expenses. Forms will be available in the Medical School Financial Aid Office (Room 1C255) or in the Financial Aid Office on main campus. You must submit this request to the Program Manager for Professional Schools.
- Complete an Online Student Loan Entrance Counseling (one for the Federal Direct Stafford Loans and one for the Federal Perkins Loan).
- Complete a Master Promissory Note for, Direct Stafford subsidized and/or unsubsidized loans and Graduate PLUS loan online at StudentLoans.gov.
- Complete a Perkins Loan promissory note to include your State and Driver's License number and telephone number. This promissory note will be mailed to the address listed on the Campus Information System website around the middle of July and must be return it to either the School of Medicine Financial Aid Office or the main campus Financial Aid Office within 45 days or the loan will be canceled.
- Maintain a copy of all documents for your personal file.
