Welcome to the Univerisity of Utah Graduate Medical Education Online Verification System

As of April 2009, University of Utah Graduate Medical Education Office will no longer provide training verifications by facsimile.  All requests for verification of internship, residency, or fellowship training must be submitted via our new online system.  There will be a charge of $15 for each verification request, payable via credit card (Visa or MasterCard).

This action has become necessary due to the increased cost and the implementation of fees we commonly incur while credentialing our own incoming interns, residents and fellows.  We apologize for the inconvenience.

Instructions for requesting training verification:

Please complete our secure training verification request.  E-mail the completed request, along with any additional forms you would like to have processed to gme@hsc.utah.edu.  Each request must also be accompanied by a signed "Authorization for Release of Information" form.

When we have received your request and payment, you will be e-mailed a standard verification of training (see example) and a receipt for your payment.  We will also complete and return your forms if no additional information has been requested.  Note that your $15 payment covers standard verification only.

Standard verification information includes:

  • Name of trainee
  • Training Program(s) / Specialty
  • Dates of training
  • Confirmation of successful completion of program(s) or
  • Explanation and further information if program not successfully completed
  • Current Contact information for Program / Specialty

If your request is for additional (non-standard) information, you will still be provided with a standard verification.  In these cases, we will also forward your custom form(s) to the appropriate department for completion.  The standard verification will include contact information for you to follow-up, if necessary.  There will be no additional charge for these ‘non-standard' requests, but additional time may be required for completion and follow-up will be your responsibility.

Additional / Non-standard information may include:

  • Specific privileges and/or procedure training
  • Performance evaluations
  • Specific need for Program Director signature

If we are unable to provide a standard verification (i.e., we have no record of the individual in question) you will be notified by e-mail and the $15 charge will be refunded.
