Licensing Information
USMLE Step 3 Exam
The licensure process begins in the Intern year. No later than January of the Intern year, all Interns should be registered to take Step 3 of the USMLE Exam. This will be the latest date that will allow the Intern to take and receive a passing score by June 30th of the Intern year. The fees for this exam are the responsibility of the Intern.
FCVS and Utah Medical License Application
All PGY-2s may submit a Utah Medical License application as soon as they receive one in their Welcome Packet or may start the licensing process by printing the materials off the licensing forms. There are two parts to the application: the FCVS application and the Utah Medical License Application. Both applications must be completed and sent in with the appropriate fees. If the applications are submitted by the residents, the GME Office will reimburse the appropriate fees. Residents must have their Utah medical license and their Controlled Substance by October 31st.
PGY-1s must wait until they are in their PGY-2 year to apply for their medical license. The GME Office will pay for all applications turned in between July 1-31 of their PGY-2 year. Applications turned in after the July 31st deadline will be the responsibility of the resident. Residents must have their Utah medical license and their Controlled Substance by October 31st.
Application Fees
The fees for Utah medical licensure include $420.00 for the FCVS report, $200.00 for the Utah State Medical Application fee, $90.00 for the Controlled Substance license and $60.00 for the Controlled Substance Exam fee. These fees are reimbursed by the GME Office for all applications submitted by the deadline.
Utah Medical License Renewals
All Utah medical licenses expire on January 31, 2010. The GME Office will pay the full cost of renewal for all residents/fellows who will be continuing on after December 31, 2010. The GME Office will pay half the cost for those leaving prior to December 31, 2010.
There are two ways to renew. You can use the form you received in the mail (if you have not moved). If you did not receive a renewal form in the mail, the GME Office has a generic form or you may print an online form.
The form will have renewal instructions and your "Internet Renewal ID Number" required to renew online. Here are the steps to renew online:
- Log onto www.dopl.utah.gov.
- Have your medical license number, social security number, your "Internet Renewal ID Number", and your debit or credit card ready.
- Select "Renew a License" and then "How do I…"
- Follow the instructions until you get to the page for "Designation of Contact Person For Access to Medical Records". This page should be completed with the name, address and phone number of the program coordinator and another office person from your department.
- At the end of the online renewal process, you will be able to immediately print out a confirmation of the renewal that is as valid as a license certificate.
- Bring the confirmation certificate to the GME Office. We will make a copy of the certificate for our files. The certificate will also serve as a receipt for reimbursement. The certificate can also be faxed to the GME Office to 585-2507.
If your program is exempt from the Controlled Substance and you have elected to obtain one, the GME Office will not reimburse for the renewal of this license.

Federal DEA
You must have your Utah medical license and Utah controlled substance license before applying for a Federal DEA registration. If your Utah medical license and Utah controlled substance license have been issued, applying for your Federal DEA is quick and easy using the following steps for an on-line application. As an appointed member of the housestaff at the Unviersity of Utah Hospital, you are eligible for a "fee exempt" DEA license. IMPORTANT: Fee exemption applies only to residents/fellows performing approved, assigned rotations. FEE EXEMPT LICENSES MAY NOT BE USED FOR MOONLIGHTING. Moonlighting requires a "fee paid" individual practitioner DEA license. Apply Now!
New Applications
After you receive you Utah medical license and Utah controlled substance license, you will need to apply for your Federal DEA unless you are in an except residency program. You can pick up an application in the GME office, or apply online. You will need the following information:
- Tax ID number and/or Social Security Number
- State controlled substance registration information
- State medical license information
- Credit Card (VISA, MasterCard, Discover or American Express)
Renewals
If the DEA has your current address on file you will receive a renewal notice prior to the expiration of your DEA registration. You can renew online.
If you have moved, but forgot to change your address with the DEA, you will not receive a renewal notice since mail from the DEA cannot be forwarded by the Post Office. You may still renew your DEA registration on-line, but be sure to update your address during the process. To complete your renewal, your will need all of the same information noted above.
Transfers
If you already have a federal DEA registration prior to coming to Utah, you must first obtain your Utah medical license and Utah controlled substance license and then notify the DEA of your change of address with the address change form. You will need to complete the form and fax it to the DEA along with a copy of your Utah medical license and Utah controlled substance license.
New Fellows
Fellow Requirements
All eligible incoming Fellows must obtain a Utah medical license, a Utah controlled substance license, and a Federal DEA certificate (as applicable) prior to beginning his/her fellowship at the University of Utah. The GME Office will send the appropriate Federation Credentials Verification Service (FCVS) application and a Utah licensing packet to each incoming fellow, along with instructions on processing and reimbursement upon receipt of notification of hire by the School of Medicine Department.
How to Obtain an NPI Number
In order to write perscriptions in the stat of Utah, if you are training in a University of Utah residency/fellowship program, you must have four things:
- A Utah medical license
- A utah controlled substance license
- A Federal DEA registration, registered to a Utah address
- A National Provider Identifier (NPI) Number
Due to insurance billing requirements, many pharmacies nationwide are now requiring an NPI number from any practitioner writing a perscription. NPI numbers are now being issued as a replacement for UPIN numbers. Once issued, your NPI number will stay with you throughout your career and follow you from state to state.
- Go to https://nppes.cms.hhs.gov
- Since you are a Health Care Provider, click on the first blue link, "National Provider Identifier (NPI)"
- Find the section titled, "Need an NPI?"
- Click on the blue "Click Here" and read the brief instructions before completing the application. You may ignore the instruction for a paper only application.
- When you have finished redading the instruction, click on "Go to previous page," located at the bottom of the page.
- Click on "Apply Online for an NPI."
- Review the items in Step 1 in the colum "Information Required for Individual Providers."
- Your pracice location should be listed as University Hospital (or Primary, Huntsman, or Moran...wherever you have your business mail sent)
- The Taxonomy Code: this is basically just your primary medical specialty and the on-line applicaiton includes a drop-down menu to choose from. Choose the number specific to your program.
- If you do not have a license please select number 39(Student Healthcare) and leave the license number blank.
- List yourself as the Contact Person.
- Read the information in Step 2, then proceed to Step 3, "Begin Online Application."
- Compete the application and then submit the application.
The appliation will take approximately 20 minutes to complete. You should receive your NPI number in 3 to 10 days. When you recieve your number, keep the confirmation in a safe place. E-mail or call the GME Office so we can add your number to our datebase.
