Credentialing

Your credentialing process must be complete before you can start work and before your payroll can be submitted. The GME office will verify the following:

  • Verification of Medical School
  • Verification of Previous Training
  • Verification of Previous Employment
  • Certification for ECFMG
  • Verification of Previous Licensure

For PGY-2 and Above

  • Completed Security Background Check
  • Completed National Practitioner Data Bank Report

You are required to provide the following information to the GME office prior to your start date:

  • Certificate of EduCAT Training
  • Complete Immunization Form
  • ACLS Certification
  • Receipt of USMLE Step 2 CS and CK Scores or other board scores for PGY-1, these plus Step 3

ACLS Recertification Classes and Registration

All houseofficers in the University of Utah Affiliated Hospitals System are required, at a minimum, to take and pass ACLS certification at least one time. All new houseofficers are required to provide documentation of ACLS certification prior to the beginning of their training at the University of Utah.

Immunizations

All housestaff in the University of Utah Affiliated Hospitals system will comply with the following health requirements:

  • A tuberculin skin test. Must be performed two (2) Months prior to first day of employment. If the test is positive, or prior skin tests are positive, a chest x-ray and written documentation of x-ray results is required. Documentation of test is required. The skin test must be repeated annually.
  • Tetanus and Diphtheria booster (within the last ten years - dates of vaccination sufficient documentation).
  • Rubeola (Measles) Immunity: Housestaff born on or after 1957 must provide written documentation of two MMR vaccinations given on or after 1970, after 12 months of age, at least one month apart; or documentation of blood titer or screen results showing immunity to rubeola (measles). Housestaff born before January 1, 1957, are only required to have one (1) MMR. Physician documentation of having had measles is also acceptable.
  • Rubella (German Measles) Immunity: Housestaff must provide documentation of receiving one rubella immunization given after 12 months of age. Physician documentation of having had german measles is also acceptable.
  • Hepatitis B Vaccine - OSHA blood borne pathogen standard requires that any employee who may have occupational exposure to blood or other potentially infectious materials as a routine part of their responsibilities and are at risk for Hepatitis B must complete a series of vaccinations for Hepatitis B, document that he/she has been vaccinated or is immune, or sign a Hepatitis B vaccination declination.
  • Chicken Pox - Written documentation of two chickenpox immunizations given on or after 1995 and at least one month apart, if given ate age 13 or greater. One chickenpox immunization is generally considered adequate if given under age 13. Documentation from a physician or a reliable history from the resident or an immediate family member is also acceptable.

Drug Testing

University of Utah and University of Utah Health Sciences Center have a "Drug Free Workplace" policy and a drug testing policy which authorizes drug testing for employees and students.

Please be aware that your acceptance of residency training at this institution obligates you to comply with all existing policies and procedures as well as new policies and procedures as they are developed and implemented.

You will receive a drug screen during your orientation.
