CME Credit Information
To receive CME credit for a live broadcast, please complete the following steps:
- View the live broadcast.
- If you have any questions that you would like to ask the speaker during the broadcast, e-mail them to imgr@hsc.utah.edu.
- Complete the Attendance Form (pdf) and Evaluation Form (pdf)
The evaluation form is completed only twice a year. Please do not put your name on the Evaluation Form. The form is available in PDF format. PDF forms require acrobat reader and can only be printed to be filled out. If you do not have this program on your computer, you can download it by clicking here. You can print the Web page format and print it out and fax it. - Return the completed forms to the department within 1 (one) hour of the conclusion of the live broadcast. Any form returned after that time will not be accepted. The forms may be sent to the department in one of two ways:
- E-mail it to imgr@hsc.utah.edu
- Fax it to (801) 581-5393
- Contact the CME Office at the University of Utah School of Medicine by phone at (801) 581-8664 or by e-mail at cme.office@hsc.utah.edu to receive your certificate or transcript for the CME credit. Please allow the UUCME office at least 1 month to process the credit requests.
Accreditation:
The University of Utah School of Medicine is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians.
Designation:
The University of Utah School of Medicine designates this educational activity for a maximum of 1 AMA PRA Category 1 Credit(s)™. Physicians should only claim credit commensurate with the extent of their participation in the activity.

Viewing of the seminar online live broadcasts requires RealPlayer. If you do not have this program on your computer, you can download the free version by clicking here. In addition, the 1-hour video files are large and require a high-speed internet connection for smooth viewing.
Faculty Disclosure:
The University of Utah Office of CME adheres to ACCME Standards regarding industry support of continuing medical education. Disclosure of faculty and commercial sponsor relationships will be made known at the activity. Speakers are also expected to openly disclose intent to discuss any off-label, experimental, or investigational use of drugs, devices, or equipment in their presentations.

Participants are expected to utilize their own expertise and judgment while engaged in the practice of medicine.
