Registration:  

The registration fee includes attendance at all three (March 12-14, 2010) days of conference sessions, the course syllabus, and a daily continental breakfast.

If cancellation of your registration is necessary, you may email Kevin Dustin at kevin.dustin@hsc.utah.edu, or call 801-581-7803.  Cancellations made by January 23, 2010, will be accepted subject to a cancellation fee of $50.  Confirmed registrants who do not attend the conference or who cancel after January 23, 2010, are responsible for the entire registration fee.  Substitutions, however, may be made at any time.

Special Assistance:  The University of Utah seeks to provide equal access to its programs to people with special needs. If assistance is required, please call 801-587-3450. Arrangements should be made at least two weeks in advance.
