Program Description:

The University of Utah, McGill University and Venous Disease Coalition (VDC) have collaborated to provide this quality improvement initiative to (1) target specific areas of educational need with regard to Post-thrombotic syndrome (PTS), (2) utilize the identified areas of need to develop a CME/CE PTS educational curriculum, and (3) measure educational outcomes based on follow-up survey data at 90-days post-educational intervention.  Educational platforms include a certified live webcast, certified web-based modules (3), and a supplemental patient education brochure.

Target Audience:
Our target audience reflects our multidisciplinary approach: community and hospital-based physicians, basic and translational scientists, pharmacists, physician assistants, nurses, administrators, and allied health professional who have an interest in thrombosis diagnosis, treatment, and prevention.
Identified Educational Need:
  1. Patients are largely unaware of post-thrombotic syndrome (PTS) as a potential long-term complication following deep vein thrombosis (DVT).
  2. Health care providers are largely unaware of post-thrombotic syndrome (PTS) as a potential long-term complication following deep vein thrombosis (DVT); therefore, possible preventive measures such as patient education, proper leg elevation, and elastic compression stockings remain underutilized.
  3. While there is a wealth of information for DVT, there is currently no established curriculum for educating both patients and providers about PTS.
Identified Educational Learning Objectives:
  1. Develop a systematic method for providing PTS education to at-risk patients (via pharmacy-managed anticoagulation clinics, vascular lab, and primary care clinics).
  2. Develop practical tools for health care providers that emphasize prevention, screening, and treatment for PTS.
  3. Develop and implement a PTS educational curriculum for both patients and health care providers.
Desired Outcomes:
  1. Increased patient awareness of PTS as a potential long-term complication of DVT.
  2. Increased health care provider awareness of PTS as a potential long-term complication of DVT and increase utilization of prevention measures and routine screening for PTS.
  3. Systematic and routine use of the PTS educational curriculum (patient educational materials and health care provider online learning modules and tools).
