Online Course Evaluation Instructions
- Go to the Login page: https://secure.uuhsc.utah.edu/som/evals/. Type in your 8-digit Student ID number, and click on "Login."
- When you login, the next page will show a list of evaluations you still need to complete. The date listed next to each evaluation is the date the course ends. You have one week after this date to complete the evaluation.
- Find the evaluation you want to complete, and then click on "Enter Responses."
- Read the instructions on the top of the evaluation carefully.
- When you are finished, click on "Update Responses."
IMPORTANT: This does not submit your evaluation to he Dean's Office. This saves your responses so you can complete the evaluation at a later date.
- You can login and change or add to your responses at any time the evaluation is available.
- When you are completely finished with an evaluation and would like to submit your evaluation to the Dean's Office, click on "Close Eval."
- You should see a message that reads "Your evaluation was successfully submitted." The completed evaluation will be taken off your list of pending evaluations.
Points to Remember
- If you have problems submitting your evaluation online, print the evaluation with your responses and bring it to 1C102 SOM.
- Course evaluations are required of all students, including those also assigned to do Group Evaluations for that course.
- Evaluations are completely confidential and anonymous. Comments are not linked to each other or to your name, so putting "see above" is useless.
- Professional, constructive feedback is encouraged. Comments are read and taken seriously by faculty and members of the administration. Comments are not edited for spelling or usage.
- Evaluations will be posted online within the first two weeks of class. Feel free to begin working on them on a daily basis.
