Within each study announcement, you can click on "click here for more information" to access a consent form. The consent form will provide you with more in-depth descriptions of the studies. We ask that you review it prior to agreeing to participate in any of the studies.
Once you review the consent, you may complete the one-page AHC Registry online and submit it to us right away. You may also view and print copies of the AHC Medical Questionnaire and AHC Daily Episode Log.
