Pre-requisites for ApplicationThe following is a list of materials to include with your University of Utah Department of Neurology Residency application.

USMLE Certified Transcript of Scores: Steps 1 and 2 – both CS and CK are required before starting your PGY1 intern year, for the PGY2 year, and Step 3 is required for the PGY3 year and higher.
Applicants must register with both NRMP and ERAS (Electronic Residency Application Service) to participate in the services of each.
In addition, if you are:
An MD, then it is required that you additionally submit:
- Letter from your current program director
- Notarized proof of graduation from medical school with date of graduation
A graduate of a medical school outside the USA, then it is required that you additionally submit:
- Copy of your valid ECFMG certificate
- Signed agreement to obtain a J1 Clinical Visa (if applicable)
