New Gynecology Patients
Please bring the following items to your first visit:
- Billing and Collections Payment Policy (Print and sign.)
- New Gynecology Consult form (Print and complete.)
- Copies of any medical records you have from other physicians regarding the problem for which you are being seen.
- Referral: If your insurance company requires a referral from your primary care physician, please bring the referral with you to your first appointment.
