New Infertility Patients
Please bring the following items to your first visit:
- Your spouse
- Copies of any medical records of previous work-up regarding infertility that you or your spouse have completed.
- Referral: If your insurance company requires a referral from your primary care physician, please bring the referral with you to your first appointment.
