How to Apply

Thank you for your interest in our Occupational and Environmental Medicine residency program.

Applicants selected for the program are usually admitted to one of two tracks:

  • Applicants may be admitted into the two year program that includes the Master of Occupational Health (MOH) degree after completing at least one clinical year of training in an ACGME-accredited residency program. 
  • Applicants already holding an MPH/MOH degree, or equivalent, who have completed at least one clinical year of training in an ACGME-accredited residency program may be admitted to the OEM practicum phase.

We require the following documents from applicants

  • Completed Graduate School Application (link to on-line application) for MOH (PYG-2) (if applicable).  (PDF version of Graduate School Application.)

  • Apply through Electronic Residency Application Service (ERAS) with the following:

    • Personal statement

    • Curriculum vitae

    • USMLE Steps 1-3 - passed (If you are a PGY-1 trainee and have not yet completed the Step 3 exam, you must have passed Step 3 prior to starting clinical care in our program.)

  • Copy of doctorate (M.D. or D.O.) degree

  • ECFMG certificate if graduate from International Medical School

  • GRE scores if graduate from International Medical School (only for PGY-2 year)

  • U.S. citizen OR Green Card holder OR have J-1 VISA

  • Copy of PGY-1 year completion certificate (If you are a PGY-1 trainee and have not yet completed your PGY-1 year, you must present a certificate of successful completion for that year prior to beginning the PGY-2 year.)

  • Letter from program director from Internship or Residency Program:

    • NOTE: This letter must include specific rotations completed and document at least 6 months of direct patient care.

  • Transcripts (College and Medical School)

  • Deans letter (unless a primary care residency has been completed)

  • At least 3 letters of reference

  • Copy of DD214 form (military service, if applicable)

  • Utah license (or applied for one prior to starting program)

  • Controlled substance license (or applied for one prior to starting program).

  • Applied for Utah resident status for PGY-2 tuition

Candidates for this program are selected based on their preparedness, ability, academic credentials, communication skills, and personal qualities such as motivation and integrity.
