Application Checklist

Please ensure that you have completed the following steps in the application process.

  1. Completed Visiting Student Application with original signatures and school seal. (One application for each rotation for which you apply).
  2. All boxes must be checked "yes" under Section 2 of application or it will be denied.
  3. A $100.00 application processing fee is required for each 2 or 4 week rotation. Payment made through the secure credit card site.
