Online application - Once submitted, the application can’t be changed.
Be sure to review the Apply Yourself Tutorial to make sure you have included all of the required information. Make sure you use an e-mail you will have continued access to; notifications will be sent through that e-mail. Also, upload all unofficial transcripts, including University of Utah.
Transcripts - Prior to the admissions deadline: You must have an unofficial transcript from each institution from which you have earned college credit (including the University of Utah) uploaded to the online ApplyYourself application. There are instructions on this in the ApplyYourself tutorial.
If you are admitted into the MPH program: You must submit an official transcript from each institution from which you have earned college credit (except the University of Utah) to the Office of Admissions. Please see the Office of Admissions website for details on how to submit official transcripts. If you fail to submit official transcripts, you will be unable to register for classes.
Current Resume or Curriculum Vitae (CV) – Resumes and CV’s should be professional and up-to-date.
Three letters of recommendation - Required for each master’s applicant; the recommendation letters should come from persons familiar with the applicant's academic and professional capabilities, achievements, work experience and/or personal qualities—not family or friends. There is no specific format for these letters.
MPH Statement of purpose - The statement, not to exceed 500 words, please describe how you think completing this joint degree will facilitate your career goals and what you bring to this organization/position in the applicable industry in which you feel you could impact this issue.
MPP Statement of purpose/letter of intent - Describe your background, interest and qualifications and how the MPP/MPH program will help you achieve your career goals.
TOEFL international students - Spring 2014 semester, applicants must have a minimum TOEFL score of 80 iBT (550 pBT) or better, or a minimum IELTS band score of 6.5 or better. (Minimum score requirements are subject to change.) Must submit TOEFL scores: at least iBT 61 or pBT 500 or an IELTS score of at least 5.0
General Graduate Record Examination (GRE) - The GRE is strongly recommended for applicants without a terminal degree (MD or PhD) earned in the U.S., with scores no older than five years (University of Utah school code 4853, department code 0616).
Applicants who do not take the GRE will need to demonstrate adequate quantitative ability through their college coursework (completion of college algebra, calculus, or statistics with a B or better grade).
MCAT scores may be substituted only if the applicant has been accepted into the University of Utah medical school and plans to seek the MD/MPH or MD/MSPH. The medical school letter of acceptance must be attached to MCAT scores.
May be required, including international applicants. Telephone interviews may be conducted and will be considered on a case-by-case basis.

The ApplyYourself online program allows applicants to track their application by logging back into the system with your PIN and password. Applicants are able to, and responsible for, tracking the progress of their applications. (This is helpful to verify all of your letters of recommendation have been submitted.)
