Online application - Once submitted, the application can’t be changed.
Be sure to review the Apply Yourself Tutorial to make sure you have included all of the required information. Make sure you use an e-mail you will have continued access to; notifications will be sent through that e-mail. Also, upload all unofficial transcripts, including University of Utah.
Transcripts - Prior to the admissions deadline: You must have an unofficial transcript from each institution from which you have earned college credit (including the University of Utah) uploaded to the online ApplyYourself application. There are instructions on this in the ApplyYourself tutorial.
If you are admitted into the MPH program: You must submit an official transcript from each institution from which you have earned college credit (except the University of Utah) to the Office of Admissions. Please see the Office of Admissions website for details on how to submit official transcripts. If you fail to submit official transcripts, you will be unable to register for classes.
Four letters of recommendation - Four professional letters of recommendation from professors or supervisors in work and/or volunteer settings, particularly those in human services settings. It is suggested that applicants submit recommendations from persons who have been directly responsible for assessing the applicants' human service-related work/volunteer experience or academic performance. One or two letters from social workers are strongly encouraged.
Statement of purpose - In the statement, not to exceed 500 words, please describe how you think completing this joint degree will facilitate your career goals and what you bring to this organization/position in the applicable industry in which you feel you could impact this issue.
If appropriate, copies of an applicant's publication abstracts should be included in the Writing Sample-Essay-Publication section of ApplyYourself.
Autobiographical statement – Applicants should include the following: (a) why they want to be a social worker, (b) if they have a sound understanding of what social workers do and (c) how their life experiences have prepared them to understand graduate studies in social work. Critical thinking essay - To help the MSW admission committee determine the applicant’s ability to think critically about the complex and multi-level challenges in social work practice, he/she is required to write an essay after choosing one of the case studies provided. Current Resume or Curriculum Vitae (CV) – Resumes and CV’s should be professional and up-to-date.
General Graduate Record Examination (GRE) - The GRE is strongly recommended for applicants without a terminal degree (MD or PhD) earned in the U.S., with scores no older than five years (University of Utah school code 4853, department code 0616).
Applicants who do not take the GRE will need to demonstrate adequate quantitative ability through their college coursework (completion of college algebra, calculus, or statistics with a B or better grade).
MCAT scores may be substituted only if the applicant has been accepted into the University of Utah medical school and plans to seek the MD/MPH or MD/MSPH. The medical school letter of acceptance must be attached to MCAT scores.
TOEFL international students - 7.0 to 7.5, applicants must have a minimum TOEFL score of 100 iBT (600 pBT) or better, or a minimum IELTS band score of 6.5 or better.
May be required, including international applicants; telephone interviews may be conducted and will be considered on a case-by-case basis.

The ApplyYourself online program allows applicants to track their application by logging back into the system with your PIN and password. Applicants are able to, and responsible for, tracking the progress of their applications. (This is helpful to verify all of your letters of recommendation have been submitted.)
