(PhD) Practicum Forms



The Teaching and Research Practicum are only necessary for students who started the PhD program prior to fall 2018

Research Practicum Forms

The research practicum cannot be completed until after the student has passed the General Examination. The purpose of the practicum is to develop a working relationship with a Public Health faculty member to serve as the chair of the student’s supervisory committee. It also provides direction into potential dissertation topics and leads into the student’s concept proposal, which must be presented and approved at faculty meeting. The objectives form must be approved by the student’s potential chair and the practicum coordinator before a permission code will be distributed. All other forms, student evaluation from 1 & 2, and the mentor evaluation form must be submitted to the practicum coordinator for approval after the presentation of the concept proposal in faculty meeting. The research practicum will be deemed complete after the student has presented their concept proposal and completed all of the required forms. All forms must be submitted to the practicum coordinator for review and then submitted to the academic advisor. A CR grade will be awarded after the completion/submission of all forms. During the completion of the research practicum the student must compose a supervisory committee for their proposed dissertation topic (5 members).

Teaching Practicum Forms

Students should complete their teaching practicum during their first or second year of coursework under the direction of a fulltime faculty member within the Division of Public Health. The goal of this practicum is to give students experience in teaching within the Public Health Program. Permission must be granted from the faculty member teaching the course of interest and the practicum coordinator before a permission code will be distributed. This is documented through the objectives form. All other forms, student evaluation from 1 & 2, and the mentor evaluation form must be submitted to the practicum coordinator for approval. In addition to the forms, a 5 page reflection paper is required. After review and approval from the practicum coordinator, the forms must be submitted to the academic advisor to be saved in the students file. A CR grade will be awarded after the completion/submission of all forms and reflection paper. For detailed instructions please refer to the PhD Teaching Practicum Procedures.
