Application
To be considered, the applicant must have completed an accredited three-year pediatric cardiology or adult cardiology fellowship by the beginning of the fellowship program. Fellowship applicants must be board certified in pediatrics by the American Board of Pediatrics (ABP) or must be eligible to sit for the ABP general pediatric examination during the first year of fellowship training and/or board certified in internal-medicine by the American Board of Internal Medicine (ABIM). The applicant must be a US citizen, permanent resident, or hold a J-1 visa. No other visas are accepted.
Application Requirement Checklist
We accept applications through the online Electronic Residency Application Service (ERAS). All documents below must be submitted through ERAS.

- Updated curriculum vitae.
- Personal statement (one page maximum) this should detail your motivation for choosing pediatric cardiology and your future career plans.
- Copies of USMLE scores and/or COMLEX scores.
- Medical school transcript.
- Letters of Recommendation (no more than four letters)
International Medical Graduates must meet the following requirements in addition to the above:
- Copy of green card, visa (J-1 only) or documentation of U.S. citizenship. The University of Utah GME Office requires that International Medical Graduates hold a J-1 visa before the MATCH deadline. Pediatric Cardiology is not allowed to rank a foreign medical graduate unless they hold a J-1 visa. - Valid ECFMG certificate with Clinical Skills Assessment certification.
- Evidence of a minimum of 6 months of some type of clinical experience in the United States in an academic affiliated hospital as evidenced by two letters of recommendation from physicians with some type of academic affiliation with whom they have worked.
Application Timeline and Selection Procedure
We participate in the National Residency Matching Program (NRMP) for the selection of first-year fellows. Applicants apply directly to the department and register with the NRMP. Our program code is 1732153S0 (ACHD Utah).
Interview Day
- Interviews will be conducted beginning in August and ending the first week in November. Interview dates will be offered to each selected applicant and up to four to five applicants will interview per date.
- The interview day will consist of:
- Welcome and introduction to our program - Daniel A. Cox, DO, FAHA, FACC, Fellowship
- Interviews with ACHD Faculty
- Interviews with ACHD Support Staff
- Interviews with Adult Cardiology/Pediatric Cardiology/Surgical Providers
- Subsequent to the applicant's interview, all of the interviewing personnel will be asked to complete an evaluation of the applicant which includes reviewing letters of recommendation, commenting concerning the interviewee's strengths, weaknesses, and career goals as well as an overall impression, at which time an overall rating will be devised. At the completion of the interviewing period, applicants will be ranked during a meeting with the Adult Congenital Cardiology Interview Committee. The results of this meeting will be tallied and will form the basis for the preliminary rank order. Final MATCH rank list will be developed and submitted to the NRMP.
