Brian Sauer's research focuses on pharmacoepidemiology, comparative effectiveness research, medical informatics, and causal inference. His work is centered on developing and implementing innovative epidemiologic and informatics frameworks that improve research efficiency, reproducibility, and scientific impact.

His research in rheumatoid arthritis demonstrated the value of reproducible analytic workflows for generating transparent and scalable evidence. More recently, his work in transthyretin amyloid cardiomyopathy (ATTR-CM) has focused on integrating information extracted from unstructured clinical text into structured, standardized OMOP data models. This approach enables researchers to leverage open-source tools, methods, and analytic workflows developed by the OMOP community and the broader health data science ecosystem.

Dr. Sauer leads the BeyondCT initiative, an effort to architect and harmonize modern FedRAMP-compliant Microsoft Azure environments that support both Veterans Health Administration operations through the Azure High-Performance Data Analytics Platform (HDAP) and VA research through the Azure Enclave Computing Environment (VAEC). His goal is to establish a unified, cloud-based ecosystem that accelerates research, analytics, and operational decision-making across the VA and complies with Federal Data Partners Data Management and the FDA evolving standards for Real World Evidence (RWE) in regulatory decision making.

Looking ahead, his vision is to expand their BeyondCT infrastructure to integrate advanced image analytics, patient-generated health data, and next-generation artificial intelligence into reproducible analytic pipelines. By combining scalable computing resources with standardized data and modern AI capabilities, he aims to support more timely, data-driven decision-making and improve health outcomes for Veterans.
