Femoroacetabular impingement syndrome (FAIS) is a common cause of hip pain and dysfunction among collegiate athletes, and a known precursor to hip osteoarthritis. Prevention is the key to reducing rates of FAIS, but research in this area is lacking. The research team believes effective prevention of FAIS starts with an in-depth understanding of how hip anatomy is altered in three-dimensions, and the extent to which abnormal hip anatomy disrupts hip biomechanics, prior to the onset of symptoms. In this study, researchers will compare 3D hip shape and biomechanics between collegiate athletes, control subjects that do not have a history of participation in the same sports, and FAIS patients who are seen in treated in the clinic. The future impact of this work is that it will support a prospective trial to determine if application of a modified training regimen reduces the prevalence of FAIS in athlete.

  2. Compare clinical outcomes through 2 year standard of care follow-up.

This is a Global registry for patients who have undergone knee replacement with a new knee replacement design that saves both the ACL and PCL and is being performed by FORCE-TJR. We are a recruiting site for this study. FORCE-TJR is a national registry and research effort with over 25,000 patients already participating, who are undergoing total joint replacement. This specific  research opportunity allows patients to contribute to research, based on patient reported outcomes that we hope will lead to improving the overall quality of life and health for people who require a knee replacement.

DEVELOPMENT OF A METHOD TO DIAGNOSE SUBTLE SYNDESMOSIS

Our primary goal is to develop a reliable diagnostic method in patients with low-grade, subtle syndesmosis instability using conventional weight-bearing radiographs and/or weight-bearing CT scans. This should help to reduce the rate of unrecognized or misdiagnosed injuries of distal tibiofibular syndesmosis and possible avoid the sequelae of pain, ankle instability and/or the development of posttraumatic ankle osteoarthritis.

AN IMAGING FRAMEWORK FOR CLINICALLY TESTING NEW TREATMENTS TO PREVENT POST-TRAUMATIC OA

The immediate goal of the research is to test the value of a new low-cost, low-dose standing CT system for efficient early detection of both joint degeneration and elevated contact stress in ankles following an intra-articular fracture of the tibial pilon. The standing CT scanner holds promise for detecting arthritic changes earlier than other imaging modalities because of the combination of its 3D nature and ability to image joints in a weight-bearing pose. A secondary goal of the research is to enable predictive models for osteoarthritis risk based on measures of post-treatment contact stress, both to inform treatment and so that new interventions can be tested in a manner incorporating risk stratification. A prospective multi-center clinical study of patients having sustained intra-articular fractures of the tibial pilon is being undertaken. Radiographs and standing CT scans obtained at follow-up clinical visits will be collected and used to evaluate joint degeneration. Clinical outcome data will also be collected.

ASSESSING RISK OF OPIOID DEPENDENCE IN TRAUMA PATIENTS: BIOMARKERS AND ATTENTIONAL BIAS OF ABUSE

This study is designed to identify early risk factors for trauma patients who go on to become opioid mis-users. Study participants will undergo psychological testing with the Attentional Bias test, a computer test designed to identify patients who preferentially pay attention to opioid, pain, and pleasure visual cues. Patients will take this test within 24 hours of injury and at the 2 week and 6 week follow-up visits. We will also obtain blood samples within 24 hours of injury, 24 hours post-op, 48 hours post-op, and at the 2 week and 6 week follow up visits. The blood samples will be analyzed for genetic expression of biomarkers linked with opioid abuse.

Pitching-related injuries are very common, occurring in 30-74% of pitchers. Tearing of the ulnar collateral ligament (UCL) is one of the most common problems, with 25% of Major League Baseball (MLB) pitchers having undergone surgery for tears of the UCL. It is unknown whether partial tears in the ligament can heal. The purpose this study is to determine the response of the UC in professional pitchers to a single season and a single off-season. We hypothesize that the ligament will respond to activity such that during the season evidence of damage to the ligament will accumulate and during the off-season evidence of damage to the ligament will resolve.

The purpose of this study is to compare a structured home exercise program to supervised physiotherapy post-operatively after primary reverse total shoulder arthroplasty by performing a multi-center, assessor-blinded, randomized clinical trial. This study has two centers: the Departments of Orthopaedic Surgery at Washington University in Saint Louis and at the University of Utah in Salt Lake City.
