** Please contact the Visiting Students Team at visitingstudents@hsc.utah.edu if you do not hear from us regarding your documents. Please also check your spam folder. It is the student's responsibility to ensure ALL required documents are submitted per your rotation deadlines. Please also note that some rotations may require earlier submission of documents to account for additional review.**

A non-refundable application processing fee—in the amount of $145 for MD Students and $295 for DO Students for each course taken—will be charged after the student accepts an offer to rotate at SFESOM. The difference in the application processing fee for DO students is the result of MD student processing fees are subsidized as part of the relationship SFESOM has with other allopathic medical schools who offer learning experiences during the MD students’ third- and fourth-years.
The non-refundable application processing fee must be received within five business days of accepting the offer to rotate.
For MD students, pay the application fee here.
For DO students, pay the application fee here.
Students who do not submit the required non-refundable processing fee within five business days will have their registration revoked.

There are lots of changes coming to U of U Health as construction is underway! Please watch this video for a detailed look at where the construction is taking place and make sure to account for it when planning your travel to campus. Please also view this page for information regarding the TRAX construction.
-
We are anticipating an HR bottleneck in August as the fall semester gets underway and onboarding occurs throughout campus. For that reason, we are encouraging students with rotations in August and September to get their post-decision documents submitted now. Please submit your post-decision documentation in the VSLO portal sooner rather than later. Please see the sample post-decision documents above for more details.

Housing
The University of Utah does not offer housing for visiting students. The University of Utah does not endorse nor is affiliated with any particular off-campus housing provider. Other options may include the University Guest House or short-term off-campus rentals.

The SFESOM requires students to apply for our electives through VSLO. Students also need to be in the clinical phase of their training and have completed six of the CORE Clerkships (Internal Medicine, Surgery, Pediatrics, Obstetrics/Gynecology, Psychiatry, Family Medicine; Neurology may be required for some rotations) prior to the requested rotation's date to be eligible. An affiliation agreement is not required for students to participate in SFESOM electives.
**Please contact the Visiting Students Team at visitingstudents@hsc.utah.edu if you do not hear from us regarding your documents. Please also check your spam folder. It is the student's responsibility to ensure ALL required documents are submitted per your rotation deadlines. Please also note that some rotations may require earlier submission of documents to account for additional review.**
-
It may be possible to reschedule your rotations to a date listed in the course catalog if you have scheduling conflict. Please email visitingstudents@hsc.utah.edu and we'll help you connect with your department coordinator. Rescheduling rotations is not guaranteed and is at the sole discretion of the department.
-
If you application was marked as ineligible, it means you did not fulfill the basic requirements for all visiting students. These requirements include criteria such as:
- having completed all 6 mandatory clerkships prior to the start of your rotation date. The six required clerkships are: Internal Medicine, Surgery, Pediatrics, Obstetrics/Gynecology, Psychiatry, Family Medicine. Neurology may be required for some rotations.
- having passed the COMLEX-USA Level 1 or the USMLE STEP 1 exam.
- having current and valid required certification (ACLS or BLS, OSHA, HIPAA).
- receiving course credit for your rotation.
- your graduation date is prior to your scheduled rotation dates.
Extending offers and scheduling/re-scheduling in-person rotations are at the sole discretion of the hosting department. If you application was denied, it means the department did not wish to move forward with your application. Course coordinators are always your best resource regarding the course, scheduling, logistics and opportunities for visiting students. You are welcome to contact course coordinators regarding opportunities to rotate at the University of Utah.

The post-decision document process is where you, the student, upload mandatory supporting documents into the VSLO portal. Please note that these documents are crucial because they are then sent to HR as part of the onboarding process. Failing to meet the exact instructions and requirements for your post-decision documents may negatively impact and/or delay your ability to start your rotation on time. Because we have found that most students will need to resubmit several documents, please keep a close eye on your email (including your spam) and plan to resubmit requested documents in a timely way. Many of these documents carry specific timelines. In addition, we want to account for holidays and peak application times where processing times may be slower than usual. Please do not hesitate to contact us if you have NOT heard from us regarding your documents.
To minimize delays with this process, please carefully read the information below and refer to the additional resources provided, including the SAMPLE documents on our website. The Post Decision documents can be found directly on the VSLO portal and include:
- 10-panel drug screen dated within 12 months of rotation start date (substances tested must be listed).
- National Sex Offender Public website search dated within 12 months of rotation start date: hOps://www.nsopw.gov/ (screenshots will be accepted).
- National criminal background check dated within 12 months of rotation start date
- OIG search within last 12 months: hOps://exclusions.oig.hhs.gov/?AspxAutoDetectCookieSupport=1 (screenshots are accepted).
- AAMC immunization form dated within 12 months of rotation start date. Your form must be completed and signed by your provider or representative.
- Computer Account Protocol Agreement
- HIPAA Training dated within 12 months of rotation start date
- Supplemental Documents (if relevant or requested)
- University of Utah Health Attestation form
Please pay special attention to the following:
National criminal background with OIG check conducted within the last 12 months
You can check your OIG status by visiting this website (hOps://exclusions.oig.hhs.gov/) and searching the database for your name. You can take a screenshot of the result and upload it in the OIG portion of your application.
If your national criminal background check was conducted more than a year from the start of your rotation start date, it will be denied. Your national criminal background check must be dated within 12 months of rotation start date and must remain valid during the duration of your rotation.
While the University of Utah is not affiliated with any background check providers nor does it recommend any specific vendors, we have seen students successfully submit national criminal background checks conducted by Certiphi, SentryLink, OneSource, and Checkr among others. There are a number of providers and you are not limited to any specific one. What is more important is that the national criminal background was conducted within 12 months of your rotation start date and will remain valid during the dates of your rotation.
The University of Utah is also offering a 10-panel drug screen for $44 through HR Storefront. The U is also offering a criminal background check + 10 panel-drug screen for $68 through HR Storefront. These options are available at: hOps://uofu.nbsstore.net/human-resources-management

Several of our visiting student rotations are done in partnership with Intermountain Health (IHC) at various IHC facilities. These rotations may have additional requirements and/or paperwork. We encourage you to contact us if you have specific questions about these rotations.
-
At the end of the elective period, it will be the visiting student's responsibility to obtain their evaluation of performance/grade from the department faculty. If your medical school requires the evaluation to be on your school's evaluation form, be sure the evaluation form is sent with your application. If an evaluation from your school is not sent with the application, your evaluation will be completed on a SFESOM Evaluation form.
-
Students receiving an MD degree must come from a school accredited by the Liaison Committee on Medical Education (LCME).
Students receiving a DO degree must come from a school accredited by the American Osteopathic Association's Commission on Osteopathic College Accreditation (COCA). DO students are not accepted into all rotations and it is up to the hosting department whether DO students are accepted or not. It is the responsibility of the DO student to ensure the rotation they request will accept DO students.
The University of Utah does not currently require an Affiliation Agreement for Visiting Students.
However, if your home institution requires one in order for you to rotate at the University of Utah, please email visitingstudents@hsc.utah.edu with your school's Affiliation Agreement form(s) and the contact information for your institution's Affiliation Agreement manager. If an Affiliation Agreement is required by your home school, please allow up to 12 weeks, and potentially longer, for completion of the Affiliation Agreement.
Please note: An Affiliation Agreement must be completed and signed by both institutions at least two weeks prior to the start of your rotation. Failure to complete the agreement by this deadline will result in the student not being able to participate in the rotation. Application fees are non-refundable.
