The School of Medicine maintains a cooperative and collaborative relationship with the Center for Disability and Access (CDA) and the Office of Equal Opportunity (OEO) to ensure compliance with federal and state regulations. ADA Accommodation Requests
Students seeking ADA accommodations are required to complete the CDA Intake Form. Further details on how to qualify for CDA Services can be found on their website.
CDA is dedicated to serve students with disabilities by providing the opportunity for success and equal access at the University of Utah. As the designated office at the University of Utah, CDA evaluates disability documentation, determines eligibility, and implements reasonable accommodations for enrolled students as guided by Section 504 of the Rehabilitation Act, the Americans with Disabilities Act, and University policy. Once a student has met with a CDA advisor and submitted all documentation, the process to determine eligibility will take at least 10 business days to complete. Therefore, students are advised to plan ahead and apply in a timely manner. Legally, accommodations are not retroactive. Once eligibility has been determined, CDA staff will communicate directly with the ASP Directors to ensure approved accommodations are implemented. Please note that once notification of eligible accommodations is received, the SOM Curriculum Unit needs two business days to implement accommodations.
Pregnancy and Pregnancy-related Accommodation Requests
Students who wish to request accommodations due to pregnancy or pregnancy-related conditions should complete the Pregnancy Accommodation Request form and submit it to the Office of Equal Opportunity and Affirmative Action. Please note that once notification of eligible accommodations is received, the SOM Curriculum Unit needs two business days to implement accommodations.
