Complete applications received by the biostatistics program by February 1 will be considered by February 15.

Documents Required for Application

Unfortunately, we do not have control over the application fees or fee waivers. You can apply for a fee waiver through SOPHAS if you feel you qualify.

SOPHAS Fees and Fee Waivers

For Slate, the application fee is controlled by the University and there is no way for us to waive it.

Primary application through SOPHAS

  • Official transcripts for US/Canadian applicants
    • WES evaluation and unofficial transcripts for international applicants
  • GRE scores (optional)
  • Three letters of recommendation
  • Statement of purpose that identifies initial faculty mentor
  • Current Resume or Curriculum Vitae (CV)
  • English proficiency for international applicants (TOEFL/IELTS)

Secondary online application through Slate 

  • Unofficial transcripts
  • English proficiency for international students (TOEFL/IELTS)

 

Online application - Applicants must submit an application through the SOPHAS system and the University of Utah. Applicants will receive information on applying at the University of Utah during the SOPHAS application process.

Transcripts - Prior to the admissions deadline: You must have an official transcript from each institution from which you earned college credit uploaded (including the University of Utah) to the online SOPHAS application.

If you are admitted into the MSTAT program: You must submit an official transcript from each institution from which you have earned college credit (except the University of Utah) to the Office of Admissions. Please see the Office of Admissions website for details on how to submit official transcripts. If you fail to submit official transcripts, you will be unable to register for classes.

Three letters of recommendation - Required for each master's applicant; the recommendation letters should come from persons familiar with the applicant's academic and professional capabilities, achievements, work experience and/or personal qualities—not family or friends. There is no specific format for these letters.

Personal statement of objectives & goals - The statement is not to exceed 1000 words

Current Resume or Curriculum Vitae (CV) - Resumes and CV’s should be professional and up-to-date.

Program Prerequisites Outline - Required for master’s applicant; in the Slate Application there is a section that requires the applicant to enter which courses they have taken to fulfill the MSTAT, Biostatistics program prerequisites. The worksheet asks for the semester and year the course was taken, the number of credit hours, letter grade and institution which the course was taken.

Admissions Tests

General Graduate Record Examination (GRE) - May be submitted, but not required.

TOEFL international students - Applicants must have a minimum TOEFL score of 80 iBT (550 pBT) or better, or a minimum IELTS band score of 6.5 or better. (Minimum score requirements are subject to change.) Must submit TOEFL scores of at least iBT 61 or pBT 500 or an IELTS score of at least 5.0

Check Your Application Status

The Slate online program allows applicants to track their application by logging back into the system with your PIN and password. Applicants are able to, and responsible for, tracking the progress of their applications.
