
Paraneoplastic Pemphigus IgG Antibodies
Ordering Recommendation
• For initial diagnostic assessment and disease monitoring in paraneoplastic pemphigus
• For best sensitivity in initial diagnosis, order concurrently with perilesional skin biopsy, when possible to obtain, for direct immunofluorescence
• For variant presentations, IgA Paraneoplastic Pemphigus Antibody testing can be requested
• Consider ordering with IgG Desmoglein 1 and IgG desmoglein 3 Antibody Levels or Pemphigus Panel and IgG BP 180 and IgG BP 230 Antibody Levels because pemphigus vulgaris may have similar clinical features and also because serum antibodies to multiple epithelial (simple, columnar, transitional) antigens may develop in paraneoplastic pemphigus including desmoglein 1, 3; desmoplakin 1, 2; envoplakin; periplakin; BP 180 and/or BP 230

Indirect immunofluorescence - mouse liver, mouse heart, mouse bladder, and rat bladder substrates (when this test is ordered alone, monkey esophagus substrate also is included in the testing)
