Verification
Electronic Verification Request Process
To request a training verification in a University of Utah Psychiatry Residency or Fellowship Training Program, please email your request to: psychtrainingprograms@hsc.utah.edu.
Please include the following in your email:
- Requestor’s name
- Requestor’s email and phone number
- Former trainee’s full name
- The year(s) that the former trainee was in the University of Utah Psychiatry Residency or Fellowship program
- An email or mailing address (if an original signature is required), where the verification should be sent
- Please attach the Training Verification Form along with an Authorization of Release of Information form signed by the trainee

Alternatively, requests using the same process as above can be sent via physical mail to our program at the address provided here:
