• Provider-led innovations to improve operations, emphasis on enhancing provider/patient experience ​
• Up to $20k-250k per year for 2 years
• Twice annual due date - next due date on 09/08/2026
• Need to think about SUSTAINABILITY of program proposed; also helpful to have a sponsor​
