John Doane, MD, Rich Doxey, MD, and the General Internal Medicine group at the University Sugar House clinic have been awarded funding for the Team Based Care Learning Collaborative Grant. The project includes a team-based approach to the diagnosis and management of hypertension. The project will enact a protocol to screen patients for hypertension, confirm the diagnosis, and then treat appropriately. Treatment will be managed by the primary care physician, as well as pharmacists to assist with medication titration. In addition, physicians, physical therapists, and dieticians will counsel patients toward more healthful eating patterns and physical activity to improve blood pressure control. Also, physicians and dieticians will direct group sessions to teach culinary skills and other skills for a healthy lifestyle.
