State of Idaho Certification

State Certification is required in order to be considered for one of the 10 Idaho acceptances at UUSOM.

All Idaho State Certifications are processed through the Idaho State University Registrar's Office, and completed certifications must be submitted to UUSOM by the applicant's Secondary Application deadline date.

Due to application volume and processing times, it is recommended to apply for State Certification when completing the AMCAS application.

 

 

Applying for State Certification  

    • Applicants will need to provide necessary documentation required by the IRDW in support of residency.

  • All signatures must be notarized.
    • If residency is being based on a spouse or parent/guardian, their signature must also be notarized.

    • Notary services are available at most campus registrar offices and many local banks or credit unions. NOTE: Due to the COVID-19 pandemic, notarization is currently not required.

  • Deliver the completed IRDW and supporting documentation to the Idaho State University Office of the Registrar via email, postal mail or in person.
