Geriatrics Clinic Move

The Division of Geriatrics Primary Care Medical Home Clinic has moved and added additional providers. The clinic is staffed by an inter professional team of physicians, geriatric nurse practitioners, a nurse care manager, Clinical pharmacist, and social worker. Two geriatricians, Drs. Florentino (Butch) Abueg and Michelle Sorweid joined our faculty this month and are accepting new primary care patients. They join a group of four geriatricians (Drs. Tim Farrell, Fred Gottleib, Alison Schlisman, and Mark Supiano) and two geriatric nurse practitioners (Drs. Amber Barnes and Kip DeWeese).
To accommodate this growth, we have moved into new, offsite clinic space in Murray (154 East Myrtle, Suite #204) where we will be located until the renovation to Madsen Clinic is completed in a little more than a year. Dr. Abueg is seeing patients in the South Jordan Daybreak clinic during this time.
We are open to seeing new patients for ongoing primary care in the medical home – especially those who have chronic care management needs. Some of our providers will see patients on a consultative basis or to co-manage with a patient’s existing primary care provider. All patients 65 and older are appropriate to receive their primary care in the medical home. However, the team is particularly well suited to provide care for the oldest—age 85 years and older—and those who may have functional or cognitive limitations or multiple, chronic medical conditions.
